Acute methaemoglobinaemia secondary to intentional dapsone overdose.

چکیده
Methaemoglobinaemia is a rare but potentially life-threatening hemoglobinopathy. It is characterized by cyanosis that is not responsive to oxygen therapy. We report a case of acute methaemoglobinaemia secondary to intentional dapsone overdose. A 34 year old lady presented to our institution with a history of sudden onset of dyspnoea. Her level of consciousness had reportedly deteriorated dramatically during ambulance transportation and her Glasgow Coma Scale was 3/15 on arrival. She was normothermic and hemodynamically stable, but had a respiratory rate of 30 breaths per minute and was cyanosed with an SpO 2 of 83% on 15 litres of oxygen via a non-rebreather mask.

Permanent retinal damage following massive dapsone overdose.
A massive dose of 7.5 g of 4,4'-diamino, diphenyl sulphone (dapsone) taken as a suicide attempt in a patient on long-term therapy for tuberculoid leprosy resulted in permanent bilateral retinal necrosis, previously unreported side effect of this drug. The patient developed a severe haemolytic anaemia, methaemoglobinaemia, and acute renal failure requiring peritoneal dialysis. It is proposed tha...
متن کاملIntraoperative cyanosis: a case of dapsone-induced methaemoglobinaemia.
Intraoperative cyanosis is most commonly caused by hypoxaemia. The anaesthetist is required to perform a rapid series of diagnostic manoeuvres and take remedial action. Occasionally methaemoglobin, sulfhaemoglobin, or haemoglobin M, undetected preoperatively, is the cause of the cyanosis. We report a case of methaemoglobinaemia secondary to dapsone ingestion that was diagnosed intraoperatively....
متن کاملClinical outcomes of adverse cardiovascular events in patients with acute dapsone poisoning
OBJECTIVE Adverse cardiovascular events (ACVEs) account for a large proportion of the morbidities and mortalities associated with drug overdose emergencies. However, there are no published reports regarding outcomes of ACVEs associated with acute dapsone poisoning. Here, the authors retrospectively analyzed ACVEs reported within 48 hours of treatment in patients with acute dapsone poisoning and...
متن کاملBarbicidal overdose
Acute severe methemoglobinaemia is an uncommon but life-threatening condition caused by a variety of oxidizing agents commonly used in both health care and industrial settings. Thus, recognition is important as it is readily treatable. The oxygen transport is compromised as a result of abnormal levels of oxidized haemoglobin, and this leads to skin discolouration and a variety of symptoms. Diag...
